WELL APP:  The “Well App” refers to the proprietary software application(s) owned by Sponsor either as mobile applications or on a software-as-a-service basis.  The Well App is available for free to download from the App Store (iOS devices) or the Google Play Store (Android devices).

OFFER PERIOD:  The Sign-up bonus (the “Offer”) is available as an ongoing activation offer, commencing January 1, 2026 through December 31,2026 (the “Offer Period”).  

ELIGIBILITY: The offer is open to individuals who are (i) eighteen (18) years of age or older at the start of the Offer Period; (ii) Amentum Services, Inc. (“Amentum”) employees enrolled in medical benefits with Anthem HSA Gold, Anthem HSA Silver, Anthem HSA Bronze, Anthem PPO 500, BCBS HDHP AL (ATI), or BCBS PPO AL (ATI) and their medically-covered spouses; (iii) and a registered member of the Well application.

PARTICIPATION REQUIREMENTS: To participate, eligible persons must activate their Well account (the “Participation Requirements”).

OFFER REDEMPTION, PRIZE(S) AND APPROXIMATE RETAIL VALUE (“ARV”):  Sponsor will award 2,500 points to each person who has completed the Participation Requirements outlined above.  Please allow 72 hours after completion of all required actions for points to be credited to registered Well user accounts.  Approximate Retail Value of 2,500 points:  $25.00. Points redeemable in the Rewards section under the redeem tab within the Well App.  No substitutions or cash-outs unless determined in the sole discretion of the Sponsor. The ARV of the prize represents Sponsor’s good faith determination.  The determination is final and binding and cannot be appealed.  Only one prize per person will be awarded.  

All federal, state, and local taxes on prizes are the sole responsibility of participants.  Redemption of points is subject to the terms and conditions of the Well Marketplace.  Prizes are not transferable and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute prize(s) of the same ARV if advertised prizes become unavailable for any reason.  All costs and expenses not specified herein relating to the prizes are the sole responsibility of participants.

PARTICIPANT’S AGREEMENT TO ABIDE BY OFFICIAL RULES:  By entering, participant agrees that s/he has read, accepted, and will abide by and be bound by these Official Rules and agrees that any dispute with regard to the conduct of this Offer, rule interpretation, or the awarding of prizes shall be submitted to Sponsor, whose decision shall be binding and final. Additionally, it is expressly understood that utilization of the Well App requires ongoing acceptance of the associated terms and conditions. For more information, please review this page. By participating in this Offer, participant agrees to release and hold harmless Sponsor and its affiliates, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.

CHANGES TO OFFICIAL RULES.:  Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper administration of this Offer as contemplated by these Official Rules.

1. ELIGIBILITY: Sweepstakes is only open to legal residents of the 50 United States and the District of Columbia (“U.S.”) residing in the U.S., who are eighteen (18) years of age or older, and are Amentum Services, Inc. (“Amentum”) employees enrolled in medical benefits with Anthem HSA Gold, Anthem HSA Silver, Anthem HSA Bronze, Anthem PPO 500, BCBS HDHP AL (ATI), or BCBS PPO AL (ATI) and their medically-covered spouses, (collectively, “Eligible Entrant”). Employees, officers and directors of Well Dot Inc. (“Sponsor”), Cohen-Friedberg Associates, LLC (“Administrator”), and all of their respective parents, subsidiaries, divisions, affiliates, shareholders, suppliers, printers, distributors and advertising, promotional agencies, (and their respective IRS dependents, immediate family members (parents, children, siblings and spouse) and individuals residing in their same household (whether or not related) (“Promotion Parties”) are not eligible to participate or win. The Sweepstakes is (a) governed by these Official Rules, (b) void where prohibited or restricted by law, and (c) subject to all applicable federal, state, and local laws and regulations. You must have access to the Internet and a valid email address in order to enter.  By entering, you agree that you have read and consent to be bound by these Official Rules, and Sponsor’s privacy policy, available at https://www.well.co/privacy Eligibility is conditioned on the acceptance of these Official Rules and Sponsor’s privacy policy at time of entry.

2. SWEEPSTAKES TIMING: The Sweepstakes begins Tuesday, January 6, 2026 at 12:00:01 a.m. Eastern Time (“ET”) and ends at 11:59:59 p.m. ET Saturday, January 31, 2026 (“Entry Period”). Random drawing will take place on or about Monday, February 9, 2026, to select one (1) potential winner (“Winner”).

3. HOW TO ENTER:
An Eligible Entrant must create a Well account and complete five (5) actions on the Well mobile app on any days during the Entry Period. Any actions found in “New for You”, “Start a Routine”, “Suggested Reading”, or on the “Journeys” tab of the mobile app will count; in addition, manual “MyHealth” entries, completing all five (5) Health Risk Assessment questionnaires in “MyHealth”, viewing a benefit (“Helpful Resources” section on the home screen of the mobile app or “Benefits” tab on the web), “Well Guide” chats on health or benefit topics, and connecting a wearable device will count. Please allow 72 hours after completion of the actions for points to be credited to registered Well user.  No substitutions or cash-outs unless determined in the sole discretion of the Sponsor.

Limit: One (1) online entry per Eligible Entrant. The Sponsor’s designated computer server is the official time keeping device for this Sweepstakes. Proof of submitting or e-mailing an entry does not constitute proof of delivery or entry. Any attempt to submit more than the maximum number of entries using multiple/different email addresses, identities or any other methods may void that entrant’s entry. Entries generated by a robotic, programmed, script, macro, artificial intelligence, or other automated means will be disqualified. No other forms of entry are valid other than those set forth above. All entry information becomes the property of the Sponsor. By entering via your wireless mobile device (only available via participating wireless carriers), data rates may apply for each message sent or received from your handset according to the terms and conditions of your service agreement with your wireless carrier. Other charges may apply (such as normal airtime and carrier charges as well as charges for wireless Internet access) and may appear on your mobile phone bill or be deducted from your prepaid account balance. Wireless carriers' rate plans may vary, and you should contact your wireless carrier for more information on messaging rate plans and charges relating to your participation in the Sweepstakes. Mobile device service may not be available in all areas. Check your phone's capabilities for specific instructions. Sponsor is not responsible for any charges.

4. PRIZE/Approximate Retail Value (“ARV”):
      One (1) Prize (“Prize”): Pair of Season Tickets (2 tickets) to your favorite sports team. If you’re not a sports fan, you may select a pair (2 tickets) to Theater or Concert Tickets. All tickets are based on availability and may not exceed $8,000 in total value. No transportation, accommodations, or other expenses included.
      Total Approximate Retail Value: $8,000
      Prize in lieu of event tickets: $6,000 (check payable to winner). Should the winner select this option, all rights/claims to any event ticket(s) become void.

References to third parties in connection with prizes are for reference and identification purposes only and not intended to suggest endorsement, sponsorship or affiliation with Sponsor or the Sweepstakes. Winner may not substitute, assign or transfer a Prize or redeem a Prize for cash (excluding the cash offer). Winner is responsible for all applicable federal, state and local taxes, if any, as well as any other costs and expenses associated with the Prize acceptance and use not specified herein as being provided. The approximate retail value (“ARV”) of the Prize is based on available information provided to Sponsor. Sponsor reserves the right to substitute a Prize of equal or greater value if the featured Prize(s) becomes unavailable. Sponsor is not responsible for and Winner will not receive the difference between the actual value of the Prize(s) at the time of award and the stated ARV in these Official Rules or in any Sweepstakes-related correspondence or material.

WITHOUT LIMITING THE GENERALITY OF THE FOREGOING, PRIZES ARE PROVIDED “AS IS” WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, FROM SPONSOR, AND SPONSOR HEREBY DISCLAIMS ALL SUCH WARRANTIES, INCLUDING BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND/OR NON-INFRINGEMENT.

5. PRIZE CONDITIONS: Prize winner will receive their Prize via mail, express mail, or electronically as determined by the Administrator at its sole discretion. All other expenses not specifically mentioned herein are solely the responsibility of the winner. The difference between any stated value and the actual value of any Prize will not be awarded. A Prize is non-exchangeable, non-refundable and cannot be exchanged for cash, and must be accepted by the Winner as awarded. Odds of winning a Prize depend upon the number of eligible entries received. If a Prize offered becomes unavailable for any reason, Sponsor reserves the right to provide an alternate Prize of equal or greater value. Winner may be required to sign and return an Affidavit of Eligibility/Liability & Publicity Release within five (5) business days following the date of first attempted prize notification, to the extent allowed by law. Failure to comply within this time period may result in disqualification and selection of an alternate winner.  In the event that any prize notification message is returned as undeliverable or if no response to the prize notification message is received within the time period stated or if the Winner is found ineligible or does not comply with these Official Rules, the Prize will be forfeited and may be awarded to an alternate winner selected at random from among remaining Eligible Entrants submissions.

6. DRAWING: There will be a random drawing conducted by the Administrator whose decisions are final and legally binding in all respects, on or about Monday, February 9, 2026 from among all eligible entries received during the Entry Period to award the Prize.

7. GENERAL TERMS: No substitution of a Prize is offered, and no transfer of a Prize to a third party is permitted (unless at the sole discretion of Sponsor). Prize Winner will be solely responsible for all applicable federal, state and local taxes on a Prize. Winner’s first name, last initial, hometown and state will be disclosed on the Winner’s List that will be distributed upon request and as required by law. No groups, clubs or organizations may reproduce or distribute any portion of these Official Rules to its members. All entrants, as a condition of entry, agree to be bound by these Official Rules and the decisions of the Sponsor and Administrator. 8. WINNER NOTIFICATION: Potential winners will be notified by email within two (2) business days after being selected as a Winner in the drawing using the email address or phone number provided at the time of entry. If winner notification is returned as undeliverable without a forwarding address, if the Winner fails to respond to the winner notification, or if the Winner is found to be ineligible or otherwise not in compliance with these Official Rules, the Prize will result in forfeiture and an alternate winner may be selected (time permitting). Failure to reply to any Winner notification within five (5) business days may result in disqualification and an alternate winner may be randomly selected from all remaining Eligible Entries received during the Entry Period.

9. CONDITIONS: All entrants, as a condition of entry into the Sweepstakes, agree to release the Promotion Parties, participating vendors, if any, and each of their respective parent companies and advertising, promotion and public relations agencies, and each of their respective officers, directors, employees and agents (“Released Parties”) from and against any and all liability, claims or actions of any kind whatsoever for injuries, damages, or losses of any kind to persons including death or property which may be sustained directly or indirectly with the receipt, ownership, use or misuse of any Prize awarded, or for any typographical or other error in these Official Rules or the announcement of offering of any Prize. The Winner shall bear all risk of loss or damage to all portions of their Prize after each respective portion has been delivered. Sponsor makes no representations or warranties of any kind concerning the appearance, safety, or performance of any Prize. Winner grants to Sponsor the right to print, publish, broadcast and use worldwide in any media now known or hereinafter known the winner’s name, address, voice, statement, picture or other likeness, without additional review, notification or compensation, for public relations, advertising and promotional purposes as may be determined by Sponsor.

10. LIMITATIONS OF LIABILITY: Sponsor assumes no responsibility for: (a) lost, illegible, late, misdirected, or incomplete, entries or email; or for interrupted or unavailable satellite, network, server, Internet Service Provider (ISP), Web site, telephone or other connections, availability or accessibility, or miscommunications, or failed computer, satellite, telephone or cable transmissions, or lines, or technical failure or jumbled, garbled, corrupted, scrambled, delayed, or misdirected transmissions, or computer hardware or software or telephone malfunctions, failures, or technical errors or difficulties, or other errors of any kind whether human, mechanical, electronic or network or the incorrect or inaccurate capture of entry or other information or the failure to capture, or loss of, any such information; and (b) any incorrect or inaccurate information, whether caused by Web site users, or by any equipment or programming associated with or utilized in this Sweepstakes; any error, omission, interruption, deletion, defect or delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or tampering with or hacking of the Web site. Sponsor reserves the right, at its sole discretion, to disqualify any individual it finds to be tampering with entry process or operation of Sweepstakes or Web site, to be acting in violation of the terms of the Web site or to be acting in a non-sportsmanlike or disruptive manner, or with intent to threaten, abuse or harass any other person. Sponsor is not responsible for injury or damage to entrants’ or any other person’s computer related to or resulting from participating in this Sweepstakes or downloading or copying materials from or use of Web site. If, for any reason, Sweepstakes, in the sole opinion of Sponsor, is not capable of running as planned by reason of infection by computer virus, worms, bugs, tampering, hacking, unauthorized intervention, fraud, technical failures or any other causes which, in the sole opinion of the Sponsor, corrupt or affect the
administration, security, fairness, integrity or proper conduct of this Sweepstakes, Sponsor reserves the right, at its sole discretion, to cancel, terminate, modify or suspend this Sweepstakes, and determine winners from entries received prior to such action taken, or as otherwise deemed fair and equitable by Sponsor. In case of dispute, the authorized subscriber of the e-mail account associated with the entry will be deemed to be the entrant. The authorized account subscriber is deemed to be the natural person who is assigned an e-mail address by an Internet Access Provider, on-line service provider, or other organization that is responsible for assigning e-mail addresses. It is solely the responsibility of the entrant to notify Sponsor in writing if the entrant changes his or her e-mail address during the Sweepstakes Period.

CAUTION: ANY ATTEMPT BY A USER OR ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.

11. DISPUTE RESOLUTION:  Except where prohibited, by participating, entrants agree that all issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrant(s) and Sponsor, shall be governed by and construed exclusively in accordance with the laws of the State of Delaware without giving effect to any principles of conflicts of law of any jurisdiction and that: (a) any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes shall be resolved individually, without resort to any form of class action; (b) any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with participating, but in no event attorneys' fees; and (c) under no circumstances will any entrant be permitted to obtain awards for and hereby waives all rights to claim punitive, incidental and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased.

12. ELIMINATION: Any false information provided within the context of the Sweepstakes by any entrant concerning identity, email or postal address, telephone number, ownership of right or noncompliance with these Official Rules or the like may result in the immediate elimination of the entrant from the Sweepstakes. Sponsor further reserves the right to disqualify any entry that it believes in its sole and unfettered discretion does not comply with these Official Rules.

WAIVER: Entrants hereby waive all rights to: (i) claim punitive, exemplary, special, incidental, indirect and consequential damages and any other damages (whether due to negligence or otherwise), other than for actual out-of-pocket costs; and (ii) have damages multiplied or otherwise increased.

13. SEVERABILITY:  If any provision(s) of these Official Rules are held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect.

14. WINNERS LIST: For the first name, last initial, hometown and state of the Prize Winner, mail in a self-addressed, first class-stamped envelope to: “$8,000 Season Tickets Sweeps” Winners, 8429 Lorraine Road, Suite #146, Lakewood Ranch, FL 34202. Requests must be received by 2/27/26.
