“ Octapharma Plasma Premium Reduction Challenge” Rules

TERMS & CONDITIONS

SPONSOR: Well Dot Inc., 1254 Chestnut Street, Suite 304, Newton, MA 02464.

ELIGIBILITY: The Offer is open to Octapharma Plasma employees enrolled in a United Healthcare medical plan (collectively, “Eligible Users”).

OFFER PERIOD: The “Octapharma Plasma Premium Discount” Rules (the “Offer”) is available beginning 8:00 am January 1, 2025 and ends at 11:59 pm on November 15, 2025.

OFFER REDEMPTION: Octapharma Plasma will award rewards or premium discounts to every Eligible User who completes the participating actions as defined below during the Offer Period. All Eligible Users can earn a premium reduction of $750 for 2026.

 

TO PARTICIPATE:

An Eligible User must complete all three of the following:

(1) Create an account with Well. On mobile: Download the “Well Digital” app from the App Store or Google Play, or on the web, visit app.well.co. Employees must enter the email address Octapharma Plasma has on file for you, your date of birth, and your mobile phone number to activate your account.

(2) Complete a visit with your primary care provider or OB/GYN for preventive care or other health needs between November 16, 2024 and November 15, 2025.

(3) Complete the online health risk assessment with Well by navigating to MyHealth, then “Wellness”, and completing all five questionnaires.


Please allow reasonable and appropriate time periods for processing of rewards. As to wellness credits, Octapharma Plasma will be the sole distributor of such rewards. Please refer questions related to such distributions to the Octapharma Plasma  HR Support Center at 800-466-8405.

RELEASE / LIMITATION OF LIABILITY: By participating in the Offer, participants agree to accept and abide by these Terms & Conditions and agree that any dispute with regard to the conduct or administration of this Offer or the interpretation of these Terms & Conditions shall be submitted to Sponsor, whose decision shall be binding and final. By participating in this Offer, participants further agree to release and hold harmless Sponsor and its dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (collectively, the “Released Parties”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to participation in this Offer or participation in any Offer-related activity.
Sponsor is not responsible for any typographical or other error in the printing of the Offer, administration of the Offer, or errors relating to the announcement of eligibility or qualification for the Offer. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Offer, or to amend the Terms & Conditions at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Offer as contemplated by these Terms & Conditions.
